 Registration Documents and Process for Enrollment 

This document outlines the registration process and required documents for enrolling a student in Lenoir City Schools. When enrolling a student, primary parent/legal guardians should complete a Registration Packet obtained from the enrolling school and provide the following documentation. 

*Homeless students shall be immediately enrolled, even if the student is unable to produce records normally required for enrollment (i.e. academic records, immunization records, health records, proof of residency) or missed the district’s application or enrollment deadlines. Parents/guardians are required to submit contact information to the district’s homeless coordinator. 

Record of Birth 

● Birth Certificate issued by any government 

● Passport issued by any nation (translated) 

● Decree of adoption or other records issued by a court 

 

2. State of Tennessee Certificate of Immunization- Evidence of state-required immunizations or exemption as authorized by state law. Any student transferring to Lenoir City Schools from out of state must have immunizations on a Tennessee form. You may obtain this form through the Loudon County Health Department at (865) 458-2662. 

3. Recent Physical Examination - Medical examination within the last two years from any state. A well-visit physical or sports physical will suffice. 

4. Parent Photo ID ● Driver’s license 

● Passport 

● Military ID 

 

5. Proof of Residence Requirement - Please submit one of the following documents as verification. All provided documentation must be in the name of the primary parent/legal guardian and align with their photo ID. ● Utility bill 

● Lease agreement 

● Tax statement (within the calendar year) 

● Mortgage statement 

● Deed 

● Property taxes 

 

 

In cases where the documents are in the name of a person other than the parent/guardian, you will need to contact our Family Resource Center to schedule an appointment with Susan Fox (865) 986-0518. The person listed must provide two forms of proof listed above and sign a notarized letter stating that the 

parent/guardian and children are living at that address. Mrs. Fox will provide the letter. Home visits for verification will be conducted as the school deems necessary. 

6. Proof of Custody (if applicable) - These documents must be provided in the instance of divorce, legal separation, or change of guardianship from parents to another party. ● Proof of legal guardianship (If parents are divorced, a copy of the divorce agreement or parenting plan with the judge’s signature must be provided showing which parent has primary legal guardianship.) 

● The primary guardian must live within the school zone for Lenoir City Middle and Elementary Schools. Lenoir City High School has open enrollment and is not limited to school zones. 

● In instances where the biological parents were never married, a notarized statement from both parties stating they were never married and both parties are aware of the change of school. 

● Power of Attorney for Care of Minor Child Act (Tenn. Code Ann. § 34-6-302) will not be accepted unless it meets the following hardships: o Serious illness or incarceration of a parent or legal guardian 

o The physical or mental condition of the parent or legal guardian of the child is such that care and supervision of the child cannot be provided 

o Loss or uninhabitability of the child’s home as a result of a natural disaster 

Please note that an LEA is not required to enroll a student with a power of attorney stating a hardship outside the scope of the above-mentioned situations. However, the LEA may enroll a student with a properly executed power of attorney for other hardships on a case-by-case basis. LEAs may also request additional documentation demonstrating such hardships and the need for the power of attorney. (Tenn. Code Ann. § 34-6-304(b)) 

For High School Students 

Enrollment Process and Documentation Required in Addition to the Above List: 

● Name and Address of the child’s previous school 

● A copy of the withdrawal form from the student’s previous school that includes withdrawal/current grades (for students transferring after school has begun) 

● An official transcript of all courses taken for high school credit. If the student is a 9th grader, we will need proof (grade card) that the student has been promoted to the 9th grade. 

● An official request for records will be sent to the previous school once required documentation is provided. This request will require an Official Transcript, Current Grades (if after school starts), Attendance, and Discipline Records before enrolling. 

● Once records are received, an appointment will be made to meet with a counselor to create a schedule and complete the registration process. Parent and student must be present.
